Fees
My fees are £350 for an initial appointment, and £220 for follow-up appointments. These fees apply for both face-to-face and video consultations. Follow-up telephone consultations are offered when appropriate and are charged at £80.
An invoice will be sent to you following the appointment. Payment is requested within 14 days of the invoice date. Full instructions on how to make the payment are included on the invoice.
Please note that I do not accept health insurance payments and therefore you must be self-funding.
Cancellations / non-attendance
If you are unable to attend your appointment for any reason, or wish to reschedule, then please contact me on 01789 532111. I would be grateful if you could give as much notice as possible so that other patients can be booked into the cancelled appointment time. Late cancellations do not allow for this, and therefore unfortunately late cancellation fees may apply (depending on circumstances):
Non-attendance / Cancellation within 24 hours of appointment 100% of fee
Cancellation within 48 hours of appointment 50% of fee
Payment
Payment should be received within 14 days of the date of invoice.
Payment can be made in one of three ways
1. Cash or cheque at the time of appointment.
2. Cheque, made payable to Dr Darren Craddock, sent to Rother House Medical Centre, Alcester Road, Stratford-upon-
Avon, Warwickshire CV37 6PP.
3. Bank transfer – details will be included in the invoice.
